Not all equipment can be altered; there must be a reasonable gargish equivalent to convert into. In general, artifacts and other named items cannot be altered. To alter an item, simply use the craft tool appropriate for the item to be altered and select the option in the craft menu. You must be a grandmaster of the craft skill, and you must also be at a soul forge. Once an item has been altered it cannot be returned to its original form.
